Overview Join our compassionate healthcare team as an Evening and Weekend On-Call Hospice Nurse, where your expertise will provide comfort, dignity, and support to patients and their families during life's most delicate moments. This dynamic role offers the flexibility to respond to patient needs in a variety of settings, including home environments and healthcare facilities. As an integral part of our hospice care team, you will deliver specialized nursing care with a focus on symptom management, emotional support, and patient-centered planning. Your dedication will help ensure a peaceful and respectful end-of-life experience for those in your care. This role is a 7-on/7-off position. Duties Respond promptly to on-call requests for patient assessments, symptom management, and urgent care needs across diverse settings. Conduct comprehensive patient evaluations including vital signs, physiological assessments, and symptom documentation. Collaborate closely with physicians, social workers, chaplains, and other healthcare professionals to develop and adjust individualized care plans. Manage electronic health records (EHR) accurately, ensuring compliance with HIPAA regulations and medical record standards. Provide education and emotional support to patients and families about hospice services, medication management, and discharge planning. Coordinate with home health agencies for seamless transition of care and post-acute management when necessary. Document all clinical interventions thoroughly in medical records and communicate updates efficiently within the interdisciplinary team. Qualifications Valid nursing license with current registration in the state of practice. Critical care experience preferred, with additional background in hospice, palliative care, or home health nursing highly valued. Demonstrated expertise in patient assessment, symptom management, and clinical case management. Knowledge of medical terminology, health regulation policies, and electronic health records (EHR/EHR systems). Strong communication skills for client interaction and interdisciplinary collaboration. Ability to manage multiple patient cases efficiently while maintaining compassionate bedside manner. Pay:
$55.00 - $60.00 per hour
