Each assessment generally takes about 45 minutes and may include annual health reviews or post-discharge visits, providing you with variety and the flexibility to tailor your schedule.

Key Responsibilities:

Complete home health risk assessments for Medicare Advantage members Review prior and current medical histories, chronic conditions, and medication use Deliver patient education on diagnoses and medications Identify care gaps and recommend actions for resolution Documentation only; there is no creation of treatment plans or prescribing medications involved

Qualifications:

Active Nurse Practitioner license in Oregon Board Certification through AANP or ANCC Current CPR certification (BLS) MSN degree from an accredited nursing program

Preferred:
