Responsibilities:

Complete comprehensive annual home health risk assessments for Medicare Advantage members Review and evaluate patients prior and current medical histories, medical conditions, and medication regimens Provide health education about diagnoses and prescribed medications to patients in a home setting Identify care gaps and offer practical recommendations to improve health outcomes Appointments may include annual assessments and post-discharge visits No requirement for creating treatment plans or prescribing medications

Qualifications:
