GENERAL DESCRIPTION OF POSITION

Work to protect and improve the health of Umatilla County residents through prevention, education and the provision of clinical and maternal-child health services. This Public Health Nurse will work in both the Nurse-Family Partnership and reproductive health and immunization clinic. It provides prenatal and family education, case management and nursing assessments with new parents and infants/toddlers in Umatilla and Morrow County in alignment with the evidence-based Nurse Family Partnership model. It will also deliver reproductive healthcare services, administers vaccines and conducts STI screening and treatment in the clinical setting. This dual role allows you to foster deep, long-lasting client relationships in the community while maintaining sharp, hands-on clinical skills in an outpatient setting. SUPERVISORY RELATIONSHIPS

Works under the direct supervision and assignment of the Nursing Supervisor and/or indirectly under the supervision of the UCo Health Deputy Director. Supervision of employees is not a responsibility of this position but may direct and oversee the work of other team members, students and volunteers. All Public Health nurses are part of the Health Department team.

PRINCIPAL DUTIES OF POSITION

Home Visit Duties Provides home visits to women and their families eligible for the Nurse Family Partnership (NFP) Program, and other home visit programs as indicated. (EE) Completes all required initial and ongoing NFP training and follows NFP model when implementing home visiting services with clients (EE). Adheres to nursing process and the NFP model of home visitation. (EE) Carries a case load of 12-15 clients for NFP program. (EE) Supports policies, procedures, guidelines and standards of UCo Health. (EE) As requested/required, participates in community coalition in support of UCo Health home visit programs. (EE) As requested/required, promotes public awareness of all UCo Health home visit programs. (EE) Develops and maintains community relationships to support client referrals. (EE) Completes all required NFP education. (EE) Develops therapeutic relationships, utilize concepts of reflection and motivational interviewing with women and their families in a home visiting environment. (EE) Performs home visiting in accordance with appropriate model and guidelines. (EE) Follows nursing process in implementing home visit models (including NFP) into nursing practice. (EE) Assesses physical, emotional, social and environmental needs of women and their families. (EE) Assists women and their families in establishing goals and outcomes. (EE) Provides education, support and referral resources in assisting women and their families in attaining their targeted goals. (EE) Consults and collaborates with other professionals involved in providing services to women and families. (EE) Formulates nursing diagnoses based on nursing assessment and client goals. (EE) Evaluates client progress toward stated goals and UCo Health outcomes. (EE) Plans home visits in accordance with client goals and NFP outcomes. (EE) Actively engages in professional development to meet all NFP Nurse Home Visitor competency requirements. (EE) Meets with Nursing Supervisor weekly for clinical supervision. (EE) Utilizes reflective practice in supervisory sessions. (EE) Schedules joint home visits with Nursing Supervisor on as needed basis, but at a minimum of quarterly. (EE) Attends and participates in case conferences and staff meetings. (EE) Provides information to support staff for timely and accurate data input to home visiting data programs. (EE) Participates in review and analysis of the data reports for achievements and areas for improvement. (EE) Participates in quality improvement efforts. (EE) Maintains confidentiality. (EE) Documents appropriately. (EE) Establish and maintain positive, professional, respectful and productive working relationships with supervisor, other employees, and customers/clients. (EE) Understands, supports, and coaches others in the NFP vision, mission and model. (EE) Represents the UCo Health vision and mission in actions and verbally to both internal and external customers and colleagues. (EE) Provide direct home, clinic or lab services to clients in both public health clinics with full documentation: Immunization Administration of vaccines. (EE) Education about efficacy and potential side-effects. (EE) Provide tracking services for the client and community [schools, physicians]. (EE) Educate client about appropriate scheduling of vaccines. (EE) Monitor and record vaccine storage temperatures twice daily. (EE) Complete vaccine inventory and usage counts monthly. (EE) Prepare, reconcile and submit monthly vaccine report. (EE) Reproductive Health Education and counseling of reproductive health clients. (EE) Provision of birth control methods. (EE) Provide relevant health screening with referral as necessary to physician or nurse practitioner for abnormal findings. (EE) Provide on-going monitoring of efficacy and client satisfaction. (EE) Sexually Transmitted Disease Provide on-site STI screening for individuals. (EE) On-site lab services [KOH, PH, and urine specimen]. (EE) Route specimens to appropriate lab sites. (EE) Provide treatment or referral of positively diagnosed individuals. (EE) Provide preventive education to individuals and groups. (EE) Investigate and control community outbreaks. (EE) Pregnancy Screening and Prenatal Education Provide on-site urine testing. (EE) Provide full options counseling, education and referrals to medical, social and nutritional resources. (EE) Provide on-going health and psychosocial assessment and case management of prenatal clients. (EE) Laboratory Testing Competency in performing, reading and documenting point of care tests. (EE) Perform routine lab maintenance, proficiency testing, quality control and quality assurance activities. (EE) Communicate with Lab Director any problems encountered during routine patient test management, maintenance checks, and quality control testing or quality assurance deficiencies. (EE) Follow UCo Health policy, procedures, and standing orders as signed by Medical Officers or UCo Health Director. (EE) Assist with orientation of new staff. (EE) Public speaking at community/school meetings regarding public health issues. (EE) Participate in occasional after hours or weekend events as needed to include health fairs, community clinics, and public health emergencies. (EE) Establish and maintain positive, professional and respectful working relationships with supervisor, other division employees, and customers/clients. (EE)

OTHER DUTIES OF POSITION

Back-up for staff nurses at all clinic sites; travel to other public health sites is required; back-up for front desk staff; weekly cleaning of labs and exam rooms; other duties as necessary and assigned. (EE)

PROFFESIONAL DEVELOPMENT

Attend educations and training modules that encourage professional development and enhance continual learning.

ACCREDITATION DUTIES OF POSITION

Actively participate as an accreditation team member. (EE) Maintain a basic knowledge of Quality Improvement based on the Plan-Do-Check-Act (PDCA) model. (EE) Actively serve on Quality Improvement teams and/or identify processes for improvement, as assigned. (EE)

REQUIREMENTS FOR POSITION

Public Health Nurse:

PHN III (R26): Graduation with an ADN from an accredited school of professional nursing PHN IV (R28): Graduation with a BSN from an accredited school of professional nursing Hold a current and unencumbered Oregon Registered Nurse license; Valid Oregon driver's license with a satisfactory driving record. Must successfully pass a pre-employment criminal history check. Must successfully pass a pre-employment drug screen for this safety sensitive position. Ability to work with minimal supervision, ability to work with clients of varying ages, gender, race, ethnicity, religious and social backgrounds.

PREFERRED QUALIFICATIONS
