Seeking a skilled nurse practitioner to conduct home health risk assessments on a per diem basis, serving Medicare Advantage plan members throughout Deschutes County. Each assessment is approximately 45 minutes, and assignments may occur in various communities within Deschutes County, such as Redmond, Deschutes River Woods, Three Rivers, La Pine, Sisters, and the surrounding areas.

Key Responsibilities:

Complete annual risk assessments for Medicare Advantage members in their homes Assess current and prior medical history, conditions, and medication usage Educate patients on medical conditions and medications Identify care gaps and provide recommendations No treatment plan development or prescribing responsibilities

Qualifications:

Active Nurse Practitioner license in Oregon Current CPR certification (BLS required) Board certification through AANP or ANCC Minimum of 6 months nurse practitioner experience preferred MSN from an accredited nursing program Compensation for this position includes $80-$90 per completed evaluation, plus a mileage stipend and remuneration for administrative time.
