Key Responsibilities:

Conduct comprehensive annual home health risk assessments in patient homes (approximately 45 minutes per evaluation) Thoroughly review medical history, current conditions, and medication use Offer education about health status, disease management, and medications to patients and families Identify care gaps and suggest actionable recommendations for follow-up Visits are focused on assessmentthere is no requirement for treatment plan development or prescribing

Qualifications:

Active Nurse Practitioner license in Oregon Current BLS certification (CPR) Board certification through AANP or

ANCC MSN

degree from an accredited nursing program At least 6 months of recent experience as a Nurse Practitioner preferred Enjoy earning $80 to $90 per completed evaluation, with additional mileage and administrative time stipends.
