Conduct comprehensive annual home health risk assessments for Medicare Advantage plan members within Bend, Redmond, Deschutes River Woods, Three Rivers, La Pine, Sisters, and nearby areas Assess patients prior and current medical history, ongoing medical conditions, and medication use Deliver education to patients regarding their health conditions and related medications Identify gaps in care and offer thoughtful recommendations to improve patient outcomes Focus solely on evaluation; no prescribing or management of treatment plans required

Qualifications:

Active and unrestricted Nurse Practitioner license in Oregon Current CPR certification (BLS) Board certified through AANP or ANCC At least six months of recent Nurse Practitioner experience preferred Master of Science in Nursing (MSN) from an accredited institution This per diem role offers competitive compensation, with $80-$90 per evaluation, plus a mileage stipend and administrative time.
