Key Responsibilities:

Complete in-home annual and post-discharge health risk assessments (approximately 45 minutes each) Evaluate patients medical history, current conditions, and medication usage Provide education on chronic conditions and medication management Identify gaps in care and offer recommendations to support patient wellness No requirement to develop treatment plans or prescribe medications

Required Qualifications:
