A rewarding per diem opportunity is available for a skilled Nurse Practitioner to perform home health risk assessments for Medicare Advantage plan members in Columbia County, OR. This position supports individuals residing in St. Helens, Scappoose, Vernonia, and other nearby communities. Schedule and Flexibility Create your own schedule with flexibility to work shifts between Monday and Sunday Pick up shifts based on your availability Commitment options for minimum weekly or bi-weekly hours Key Responsibilities Complete comprehensive annual home health risk assessments for Medicare Advantage members in their homes Assess medical history, current health status, and medication regimens Deliver education regarding health conditions and medication adherence to patients Identify care gaps and make actionable recommendations for closing those gaps

No responsibility for developing treatment plans or prescribing medications Required Qualifications Active Nurse Practitioner license in Oregon Board Certification (AANP or ANCC) Master of Science in Nursing (MSN) from an accredited institution Current CPR (BLS) certification At least 6 months of recent NP experience is preferred Compensation & Benefits Enjoy compensation of $80-$90 per completed evaluation, plus additional stipend for mileage and administrative time.
