Hood Hospice is a mission-driven organization providing compassionate, patient-centered end-of-life care throughout the Portland metro area. We are focused on delivering consistent, high-quality care while continuing to grow and strengthen our impact in the community. We are seeking a full-time Registered Nurse Case Manager who values meaningful relationships, takes ownership of their work, and is comfortable working independently while staying connected to a collaborative team. In this role, you will manage a defined caseload, coordinate care with physicians and the interdisciplinary team, and guide patients and families through the hospice journey. You will complete routine and PRN visits, provide education and support, and ensure care is delivered consistently and thoughtfully. Success in this role requires strong clinical judgment, clear communication, and the ability to stay organized while managing multiple patients and priorities. You should be dependable, proactive, and comfortable taking responsibility for your caseload while maintaining alignment with team expectations. Key Responsibilities:
Manage a caseload of approximately 10
  • 13 patients, ensuring care is consistent, organized, and aligned with patient and family goals Perform ongoing assessments, recognize changes in condition, and adjust care plans accordingly Build strong, trusting relationships with patients and families by providing clear guidance, education, and support throughout the hospice journey Coordinate care with physicians and the interdisciplinary team to ensure seamless communication and follow-through Complete routine and PRN visits efficiently while maintaining a structured and manageable daily schedule Document care accurately and in a timely manner to ensure clarity and continuity across the care team Communicate proactively with team members, addressing changes, concerns, and updates without delay Participate in a shared on-call rotation, providing after-hours support as part of a team-based approach Take ownership of your caseload by staying organized, following through on commitments, and addressing issues before they escalate Adapt to changing patient needs while maintaining consistency, professionalism, and a steady approach to care
Qualifications:
Active Registered Nurse license in the State of Oregon Experience in hospice, home health, or a related field preferred Strong clinical judgment with the ability to assess situations and make sound decisions independently Organized and able to manage multiple patients while maintaining clarity and follow-through Clear, compassionate communicator who can build trust with patients, families, and team members Comfortable working independently while staying aligned with team expectations Reliable and consistent in day-to-day work, with a strong sense of ownership and accountability
