Key Responsibilities:

Perform comprehensive home health risk assessments for Medicare Advantage member visits (average assessment duration: 45 minutes) Review and document prior/current medical history, conditions, and medication lists Educate members and their families regarding diagnoses and medication usage Identify care gaps, offering clinical recommendations to optimize patient outcomes Conduct annual health evaluations and post-discharge visits as assigned (no treatment plan development or medication prescribing involved)

Qualifications:

Active Nurse Practitioner license in Oregon Current CPR (BLS) certification Board certification via AANP or ANCC Master of Science in Nursing (MSN) from an accredited institution Minimum of 6 months of recent NP experience preferred Your commitment and skills will be recognized with competitive compensation of $80-$90 per evaluation, along with a mileage stipend and compensation for administrative tasks.
