Key Responsibilities:

Perform annual home health risk assessments for Medicare Advantage members in their residences Complete thorough reviews of prior/current medical history, conditions, and medications Educate individuals about their conditions and prescribed medications Identify care gaps, providing practical recommendations to support continuity of care No requirement for developing treatment plans or prescribing medications

Qualifications:

Active Nurse Practitioner license in Oregon Board certified through AANP or ANCC Current CPR (BLS) certification MSN degree from an accredited nursing program At least 6 months of recent NP experience (preferred) Commitment to high standards of patient care and professional integrity In this role, youll receive $80-$90 per completed evaluation, along with a mileage stipend and payment for administrative time.
