Primary Responsibilities:

Complete thorough annual health and post-discharge risk assessments for Medicare Advantage plan members in their homes Assess and document comprehensive medical histories, existing medical conditions, and medication use Educate patients on disease management and proper medication usage Pinpoint gaps in care and offer actionable recommendations to improve patient outcomes Visits focus on evaluationno development of treatment plans or prescribing required

Qualifications:

Active Nurse Practitioner license in Oregon Board certification through AANP or

ANCC MSN

from an accredited nursing program Current CPR (BLS) certification Minimum of 6 months NP experience preferred
