Key Responsibilities:

Conduct thorough annual home health risk assessments Evaluate previous and current medical history, as well as medication use Educate patients on their health conditions and prescribed medications Identify care gaps and offer actionable recommendations Complete visits efficiently (approximately 45 minutes per evaluation) Serve diverse areas within Josephine County, potentially including Golden, Greenback, Waldo, Cave Junction, Grants Pass, and beyond Focus exclusively on assessment and educationno treatment plan development or prescribing required

Qualifications:

Active Nurse Practitioner license in Oregon Board Certification through AANP or

ANCC MSN

degree from an accredited nursing program Current CPR (BLS) certification Minimum of 6 months recent NP experience preferred This position offers: Earn $80$90 per evaluation, plus receive a mileage stipend and compensation for administrative time, ensuring your contributions are well supported.
