Post-Acute Rehab and Assisted Living Facilities, Marquis at Home, Consonus Rehab and Consonus Pharmacy. As a MDS Nurse RN, your experienced nursing skills will be utilized as you conduct and coordinate the development and completion of the resident assessments (MDS) in accordance with current rules, regulations, and guidelines, including the completion of CAAs and Care Planning. This role will cover 2 of our SNF facilities, on site on rotating days- Marquis Wilsonville and Marquis Hope Village(Canby) . It will include coordination with Direct care staff, residents/families, Physicians, healthcare providers and facility inter-disciplinary teams- as you complete the assigned MDS assessments. MDS experience recommended, not required. RNs with some long-term care experience, a strong passion for working with the senior population, and experience in Nurse Supervisor roles are encouraged to apply.
