Schedule:

Choose your own hours, Monday through Sunday Minimum weekly or bi-weekly hourly commitment Locations may include Salem, Dallas, Falls City, and surrounding areas

Key Responsibilities:

Conduct comprehensive Medicare Advantage home health risk assessments Review patient histories, ongoing conditions, and current medications Offer education related to diagnoses and medication management Highlight gaps in care and make recommendations for follow up No requirement to prescribe medication or develop ongoing treatment plans

Qualifications:

Active and unencumbered Nurse Practitioner license in Oregon Graduate of an accredited MSN program Certification from AANP or ANCC Current BLS/CPR certification At least 6 months of recent NP experience preferred
