Schedule:

Flexible shifts between Monday and Sunday Self-determined schedule creation Minimum weekly or bi-weekly hourly commitment

Qualifications:

Active Nurse Practitioner license in OR Current CPR certification (BLS) Board Certification through AANP or ANCC Minimum of 6 months of recent Nurse Practitioner experience preferred MSN degree from an accredited nursing school

Key Responsibilities:

Perform annual home health risk assessments for Medicare Advantage plan members Assess prior and current medical histories, medical conditions, and evaluate medication regimens Deliver patient education focused on health conditions and medication usage Identify gaps in care and offer recommendations to address these needs Visits may include annual assessments or post-discharge evaluations (no prescribing or treatment planning required) Each assessment is expected to take about 45 minutes
