A Nurse Case Manager applies nursing expertise to coordinate a patient's medical care and provides advocacy to help them get the best and most affordable care. Develop care plans including discharge planning, coordinate delivery among providers, and teach patients and families how to follow the plan. They may also help patients navigate insurance and other healthcare bureaucracies and access related social services. May work with patients who have chronic illnesses or other complex medical needs. Works at a hospital or other healthcare facility; may follow patients throughout the span of their treatment.

Job Description
Qualification & Requirements:
- Good standing Registered Nurse or Bachelor of Science in Nursing ( BSN ) license in the State of Pennsylvania.
- Clinical or other work experience as per PA laws and/or federal laws and regulations.
- CPR certified
- Valid Driver's License and Acceptable MVR.
- Pediatrics experience is helpful, but not necessary.
Essential Job Functions :
- Patient OASIS assessments and coordination of care.
- Health, promotion, teaching, and training of family members.
- Assess and observe patient's condition and prepare plan of care.
- Coordinate with Physicians, Physical Therapists, Occupational Therapists and other individuals in the patient's care plan.
- Prepares clinical progress notes, assists the clinical team in performing specialized procedures, equipment, materials, assists patient in learning self-care techniques.
- Completes all required documentation in accordance with the standards of the agency, and regulatory requirements.
- Implements infection control and safety practices according to agency policies.
- Adheres to OSHA guidelines.
- Any other job functions assigned by the DON.
