Services include intake, referral, outpatient therapy, psychiatric assessment, service coordination, substance abuse treatment, training and social rehabilitative services, crisis intervention, partial hospitalization, community outreach, day, and recreational services for adults with intellectual disabilities, homeless outreach, and housing program. Chartiers Center's LPNs are critical members of a multidisciplinary team and are responsible for providing direct skilled care under guidance of the Director of Residential Treatment Program.
Responsibilities include the following:
the plan of care, administering medication, maintaining current health profiles through documentation, completing medical aspects of treatment and working collaboratively with the members of the team to help our clients achieve their recovery goals. Daylight, evenings, weekends and overnights. Employees will earn a 10% shift differential to his/her rate when working the 11p•7a shift.
Minimum Qualifications:
LPN licensure with the state of Pennsylvania required One (1) year experience Valid driver's license
