Key Responsibilities:

Complete comprehensive annual risk assessments in patient homes, focusing on evaluating current and prior medical history, conditions, and medication use Provide thorough education to patients about their health conditions and medications Detect gaps in care and suggest actionable recommendations for clinical teams Visits will not require you to develop treatment plans or prescribe medications Assessments are generally 45 minutes in duration

Qualifications:

Active and unrestricted Nurse Practitioner license in Pennsylvania Board Certification from AANP or ANCC Current CPR certification (BLS) MSN from an accredited nursing program Minimum of six months recent experience as a Nurse Practitioner is preferred This role not only allows for earning $80-$90 per evaluation along with a mileage stipend and additional compensation for administrative time, but also lets you make a direct impact in community health while maintaining a schedule that suits your life.
