Key Responsibilities:

Conduct comprehensive annual risk assessments during in-home visits (approximately 45 minutes each) Review medical history, current health conditions, and medication usage Educate patients about their diagnoses and medications during each visit Recognize and address gaps in care, and recommend solutions to promote patient well-being Visits may vary: annual health assessments and post-discharge evaluations No responsibility for developing treatment plans or prescribing medications

Qualifications:

Active Nurse Practitioner license in Pennsylvania Board Certification through AANP or ANCC Current CPR (BLS) certification Master of Science in Nursing (MSN) from an accredited program Minimum six months recent NP experience is preferred Enjoy compensation of $80$90 per evaluation, plus a mileage stipend and payment for administrative time.
