Job Description:
I. PURPOSE OF JOB
Provides direct nursing care to hospice patients. Is responsible for admissions of patients to service, and as needed routine SN visits and death visits. Admission nurse also responsible for coordination of other professional and para-professional services on admission if needed. This will be performed with respect to the appropriate specific Chronological Age, Developmental Age or Social Maturity of the patient.II. ESSENTIAL FUNCTIONS
- Promotes patient safety through adherence to policies, procedures, guidelines, Joint Commission standards, and state and Medicare regulations. This includes following physician orders. Demonstrates excellent clinical competency at all times according to departmental and program standards as well as the standards of each discipline, 100% supervisory visit compliance and completes competencies on time. Demonstrates patient case management of admissions by assessing, planning, implementing, creating initial care plan, and documenting patient care needs according to department guidelines in order to achieve quality outcomes. Maintains compliance with documentation standards. Communicates and collaborates with patients, families, physicians, and others to provide coordination of care and customer service. Demonstrates fiscal responsibility by controlling costs, and enhancing revenue.III. JOB CRITERIA AND PERFORMANCE STANDARDS
Promotes patient safety through adherence to policies, procedures, guidelines, Joint Commission standards, and state and Medicare regulations. Implements treatment directly. Treats patients prescribed amount of times following orders and established treatment plans. 2. Completes all aspects of patient care within established time frame and following established criteria 3. Supervises aides as needed for routine visits. Instructs and manages aides in treatment techniques as necessary. 4. Participates in compiling P.I. Monitor data and performing chart audits as directed by supervisor. 5. Makes an initial evaluation taking into consideration the admission policies of the department. Demonstrates excellent clinical competency at all times according to departmental and program standards as well as the standards of each discipline 1. Completes departmental competencies annually. Demonstrates clinical skill in supervisory visits. 3. Practices excellent bag technique, car management, and infection control practices. 4. Establishes a nursing diagnosis with goals and objectives and regularly re-evaluates the plan as the patient's needs and conditions change. 5.Performs Nursing Procedures Which Include But Are Not Limited To:
1.GENERALSKILLED SERVICES
1. Vital signs to include C/P Assessment 2. Bathing. 3. Colostomy care and irrigations, urostomy care, tracheostomy care. 4. Wound care, including irrigations and specialized Dressings. 5. Catheter insertion/male and female, plus suprapubic tube changes. 6. Bladder irrigation and instruction in self-catheterization. Flush bili tubes, ureteral tubes. 7. Set up medication schedule, supervise medication effects, administer medication injections - sub-cutaneous and intramuscular per Medication Administration Policy. 8. Chest tube irrigation and redressing. 9 .Skin care - preventive and healing measures 10.Enemas. 11.Instruction in care of feeding tubes and pump. 12.Dietary instruction. 13.General health and hygiene supervision. Preventive skin measures. 14.Collection of specimens for labs. 15.Oxygen and mini-neb instruction related to use, care of equipment and safety precautions 16.Fall Prevention measures 17.Assessment of equipment needs and supplies, demonstration of use, care and safety precautions 18. Other clinical procedures as approved. 6. Maintains Adequate Clinical Records within established departmental guidelines. Writes patient care plan and continually updates. Records home visits and conferences with physicians, therapists, aides or consultants. Demonstrates patient case management by assessing, planning, implementing and documenting patient care needs according to department guidelines in order to achieve quality outcomes. 1. Develops treatment plans, including goals and methods to achieve identified goals. 2. Modify plans, goals and methods in writing as necessary during length of treatment. 3. Organize and write protocols and procedures for identified patient programs following established criteria. D. Develops appropriate home or community programming to maintain and enhance the performance of the patients in their own environment. 1. Monitors patients' response to therapy and modifies treatment as indicated to attain goals. 2. Reviews the quality and appropriateness of the total services delivered and of individual therapy programs for effectiveness and efficiency. Exports/synch same day of visit. Communicates and collaborates with patients, families, physicians, and others to provide coordination of care and customer service . 1. Coordinates treatment plan with patients, family, significant others and other members of the family. 2. Communicates, consults and shares pertinent information with co-workers and supervisors according to established channels and lines of authority at all times. 3. Communicate evaluation findings to patients, family, and significant others as appropriate. 4. Recommends already established community or hospital programs to patients and/or families as appropriate during treatment. F. Demonstrates fiscal responsibility by controlling costs and enhancing revenue. 1. Always monitors authorizations for services and provides service within those authorizations. 2. Obtains socio-economic information and past medical history of patient and family to assess possible implications affecting plans for care. Uses supplies accordingly with no excessive use.Job Qualifications:
V. QUALIFICATIONS
- A.Education:
Registered Professional Nurse with current license with Bureau of Professional & Occupational Affairs in the Commonwealth of Pennsylvania. B.Experience:
One year recent medical-surgical experience or community health experience desired. Two years of experience preferred. C.Other Skills:
Current CPR certification. Current drivers' license and auto insurance. Ability to drive a car in inclement weather. Auto in good working condition. Professional attire for patient care. Basic Computer skills.Similar jobs in Doylestown, PA
