00 per hour Flexible Schedule Reliant Home Health Agency is seeking a compassionate, dependable, and detail-oriented Registered Nurse (RN) to join our home health team. The Registered Nurse will provide hands-on skilled nursing care, participant assessments, care planning, clinical oversight, and supervisory visits in participants' homes. The RN will help ensure that each participant receives safe, appropriate, and high-quality care in accordance with their individualized plan of care. This position requires strong clinical judgment, excellent documentation and communication skills, and the ability to work independently in a home and community-based setting. Primary Responsibilities Conduct initial and ongoing nursing assessments Complete required reassessments and nursing visits Develop, review, and update individualized plans of care Monitor participants for changes in condition and communicate concerns appropriately Evaluate participant needs and ensure services remain appropriate Provide nursing oversight of participant care Perform supervisory visits as required Review caregiver performance and ensure care is being provided according to the participant's plan of care Educate participants, families, and caregivers regarding health conditions, medications, treatments, and safety needs Coordinate and communicate with participants, families, caregivers, physicians, and other members of the healthcare team Report significant changes in condition, incidents, or clinical concerns to appropriate parties Maintain accurate, timely, and complete clinical documentation Follow agency policies, infection-control procedures, HIPAA requirements, and applicable nursing standards and regulations Skilled Nursing Responsibilities Administer medications as ordered and monitor participants for effectiveness and adverse reactions Administer prescribed injections and other ordered treatments Perform wound assessments, wound care, and dressing changes Provide catheter care and other ordered urinary-care procedures Monitor and document vital signs, including blood pressure, pulse, temperature, respirations, and oxygen saturation Perform head-to-toe and focused nursing assessments as clinically appropriate Monitor blood glucose and provide diabetic care and education as ordered Assess pain and monitor participant response to treatment and interventions Monitor for changes in physical, cognitive, or behavioral condition Provide medication education and reinforce medication-management instructions Provide disease-management and health education to participants and families Carry out physician/provider orders within the RN's scope of practice Communicate significant clinical findings or changes in condition to the physician/provider and appropriate members of the care team Respond appropriately to urgent clinical concerns and determine when escalation or emergency intervention is necessary Document all skilled nursing services and participant responses accurately and promptly Qualifications Current and unrestricted Pennsylvania Registered Nurse (RN) license Current CPR/BLS certification Previous nursing experience required Home health, home care, community-based, or long-term care experience preferred Strong nursing assessment and clinical judgment skills Experience providing hands-on skilled nursing care Excellent verbal and written communication skills Strong documentation and organizational skills Ability to work independently and manage multiple participants/cases Comfortable traveling to participant homes for skilled nursing and supervisory visits Professional, dependable, and compassionate when working with participants and families Ability to maintain participant confidentiality and follow HIPAA requirements Schedule Full-Time Flexible schedule based on participant and agency needs.
