Key Responsibilities Provide nursing care according to each patient's plan of care, provider orders, and your professional scope of practice. Monitor patient conditions and promptly communicate changes to the appropriate healthcare team members. Administer prescribed medications and treatments. Educate patients and caregivers about care instructions, medication routines, and home safety. Complete accurate and timely clinical documentation. Collaborate with patients, families, and healthcare professionals to support continuity of care. Follow infection prevention practices and protect patient privacy. Use agency systems to document care and complete required visit verification. Additional RN Responsibilities Conduct nursing assessments and identify patient care needs. Develop, implement, and evaluate nursing care plans. Coordinate care with providers and other healthcare team members. Provide clinical supervision and guidance as assigned. Qualifications Current authorization to practice as an RN in Pennsylvania. Complete initial nursing assessment within 24 hours. Current CPR/BLS certification. Strong communication, documentation, and organizational skills. A compassionate, professional, and dependable approach to patient care. Ability to travel to assigned patient homes. Ability to complete required employment screenings and credential verification. Comfort using electronic documentation and mobile applications. Preferred Experience Previous home health nursing experience. Experience using Alora Home Health software and/or HHAeXchange . Familiarity with electronic visit verification (EVV), electronic clinical documentation, and scheduling systems. Schedule and Service Areas Full-time and part-time opportunities are available.
