Key Responsibilities:

Conduct comprehensive home health risk assessments for Medicare Advantage plan members Review patients medical history, current medical conditions, and medication usage Educate patients on managing health conditions and understanding their medications Identify and report care gaps, recommending solutions No prescribing or treatment plan development is required

Qualifications:

Active Nurse Practitioner License in Pennsylvania Board Certification through AANP or ANCC Current CPR (BLS) certification MSN degree from an accredited nursing program At least 6 months current Nurse Practitioner experience preferred This role offers competitive compensation of $80-$90 per evaluation, plus a mileage stipend and paid administrative time.
