Job Description

Heart to Heart is a leading provider is services to individuals with intellectual and developmental disabilities throughout New Jersey and Pennsylvania. We are currently looking for a Registered Nurse who aligns with the mission and vision of Heart to Heart to work in our programs and provide new hire training, LPN supervision, medication training. Must be at least 18 years of age or older Two (2) years working with individuals with intellectual and developmental disabilities (preferred) Two (2) years of nursing experience Valid Pennsylvania RN License in good standing with Board of Nursing Non Probationary Driver's License High School Diploma or Equivalent Provides oversight and support for all medical and health care needs of individuals Conducts training for management and community support staff Provides oversight/supervision of LPN's working in our group homes Provides nursing services to individuals by identifying and reporting on health needs of individuals through observation of their physical, social and emotional needs as well as review of their medical records Administers medication and documents in compliance with Heart to Heart policies which conform to ODP guidelines Other duties as assigned by Chief Program Officer Orients and trains new community support staff to the medical needs of specific individuals with whom they will be working Ensures that policies and procedures that guide and support the provision of care are implemented and the recommendations to improve performance of care are implemented within Heart to Heart Reviews and maintains the system of clinical records, reporting, reporting forms and service data, ensuring that all documentation is timely, appropriate, accurate and within agency guidelines
