This full-time position involves providing management and delivery of patient care plans in Western PA and surrounding areas. The role requires on-call availability on weekends and local travel to patient locations. The successful candidate will have an associate degree, an RN license in the state of work, and at least one year of RN experience. Key Responsibilities

• Lead and collaborate to provide management and delivery of patient care plans
• Support patients in Western PA and surrounding areas
• Spend ~20% of the day driving to/from patient locations
• Consistently meet critical deadlines for charting
• Self-manage time and schedule Schedule and Shift Details
• Schedule 4
• 10 hour days, either Friday
• Monday OR Saturday
• Tuesday.  4th day is an admin day  2 holidays per year that fall on the weekend Travel
• Local travel requirements Qualifications Required Associate degree or higher 1+ years of experience in ANY of the following: Clinical Documentation Clinical Informatics Clinical Quality 1+ years of experience in ANY of the following: Electronic Medical Record (EMR) Other EMR Practice Partner EMR Valid Registered Nurse (RN) in Pennsylvania (PA) ALL of the following valid licenses/certifications: American Red Cross Advanced Life Support (ARC ALS) American Heart Association Advanced Cardiac Life Support (AHA ACLS) Are you okay with the required travel for this position?
