Registered Nurse

Responsibilities:

Conduct comprehensive nursing assessments of patients//' physical, emotional, and psychosocial needs. Develop, implement, and evaluate individualized care plans in coordination with the interdisciplinary team (including physicians, therapists, social workers, and aides). Provide direct skilled nursing care, such as wound care, medication management, IV therapy, symptom management (especially pain and comfort in hospice), and patient/caregiver education. Offer education and emotional support to patients and their families/caregivers based on individual needs. Supervise LPNs/LVNs, home health aides, and other support staff. Participate in on-call rotations, admissions, or urgent response as needed (common in hospice roles). Coordinate care activities, including visit scheduling and documentation to ensure compliance with regulations. Monitor patient progress, respond to changes in condition, and adjust care plans accordingly.
