Nurse Practitioners Hours:
Per Diem ( Flexible schedule) The Nurse Practitioner will be responsible for, but are not limited to: Conduct visits to patients on palliative care in accordance with local, state and federal regulations. Communicate visit findings to designated members of the interdisciplinary team, including the National Director of Palliative Care. Facilitate access to appropriate support models of care (i.e., hospice, rehabilitation, on-going aggressive treatment). Consult with other members of interdisciplinary team to contribute to the patient's plan of care. Prepare and maintain accurate patient records, charts and documents. Assist in development of clinical practice guidelines/standards in support of quality palliative care as needed. Ensure collaboration with patients, caregivers, medical directors and members of IDT.
Education:
Master's Degree in
Nursing Professional Experience:
Must have a Nurse Practitioner's license in PA Must have 2-4 years experience working within hospice or related field Palliative care experience preferred Must have a Master's Degree Must have a valid driver's license and a reliable source of transportation Must be willing to travel Ascend Palliative Care - is a growing part of the Ascend family of Healthcare Companies.
