Registered Nurse Assessment Coordinator Job Details:
Pay Rate:
Starting at $60/hourLength:
4 weeks minimum with possible extensionSchedule:
Monday-Friday, Day Shift —NO WEEKENDS
Remote Option:
2 Days Per Week on Monday and/orFriday Shift:
8:00 AM-4:00 PM or 9:00 AM-5:00PM Rate:

4 weeks minimum with possible extension Coordinate timely and accurate RAI/MDS assessments, CAAs, and documentation. Develop and update individualized resident care plans. Collaborate with the IDT on assessments, care planning, and MDS meetings. Audit clinical documentation for accuracy, compliance, and proper coding. Ensure accurate MDS coding and documentation to support reimbursement.Registered Nurse Assessment Coordinator Job Qualifications:
Current, unrestricted PA RN license or valid multistate RN license with privilege to practice in Pennsylvania 2-year college degree preferred Resident Assessment Coordinator certification preferred At least 1 year of long-term care experience Knowledge of MDS/RAI and Care Plan processes Knowledge of PPS, State Case Mix, and PDPM Strong verbal and written communication skills Ability to work independently and collaborate effectively with the interdisciplinary teamFacility Details:
