The RN Assessment Coordinator is responsible for conducting thorough assessments of residents' medical and care needs, developing care plans, and ensuring accurate documentation for reimbursement purposes. This position plays a lead role in coordinating care delivery, monitoring resident progress, and maintaining compliance with regulatory requirements. As a key support member of the skilled nursing leadership team, candidates will be required to exhibit excellent customer service behaviors and interactions through effective communication with team members, residents, and family members.
