Nurse Assessment Coordinator (MDS/RNAC) Aventura at Prospect Park Aventura at Prospect Park is seeking a knowledgeable, detail-oriented, and compassionate Nurse Assessment Coordinator to join our team. The ideal candidate will play an essential role in coordinating the Resident Assessment process and ensuring accurate, timely, and compliant documentation that supports quality resident care.
RESPONSIBILITIES
Coordinate and complete the MDS assessment process in accordance with applicable regulations and facility requirements. Ensure MDS assessments are accurate, complete, and submitted within required timeframes. Coordinate with nursing, therapy, dietary, social services, and other interdisciplinary team members. Participate in interdisciplinary care plan meetings and ensure resident care plans reflect current assessments. Review clinical documentation to identify changes in residents' conditions and ensure appropriate interventions are documented. Monitor and track MDS schedules, assessments, and supporting documentation. Maintain compliance with federal and state regulations related to MDS, care planning, and resident assessments. Communicate assessment findings and resident needs with the interdisciplinary team. Assist with identifying and addressing documentation gaps. Support quality assurance and performance improvement initiatives. Maintain confidentiality of resident information and comply with HIPAA requirements. Provide education and guidance to staff regarding MDS processes and documentation as needed.
QUALIFICATIONS
Current RN or LPN license in the applicable state. At least 1 year of
MDS/RNAC
experience in a skilled nursing or long-term care setting. Strong knowledge of the MDS 3.0 process and applicable CMS regulations. Experience with care planning and interdisciplinary team coordination. Strong clinical assessment, documentation, and organizational skills. Excellent written and verbal communication skills. Strong attention to detail and ability to meet deadlines. Knowledge of Medicare and Medicaid requirements preferred. Ability to work independently while collaborating effectively with an interdisciplinary team. Commitment to providing compassionate, resident-centered care.
