Job Description

Job Overview AscendEdge Careers is seeking experienced Registered Nurses (RNs) with Home Health and/or Hospice experience for opportunities throughout Pennsylvania. This is an on-the-road, field-based nursing opportunity for RNs who are comfortable traveling within an assigned territory to provide care to patients in their homes and community-based settings. If you have experience in Home Health, Hospice, Palliative Care, Visiting Nursing, or Community-Based Nursing , we want to hear from you. What You'll Do As a field-based RN, you may: Provide skilled nursing care to patients in their homes Complete comprehensive nursing assessments and reassessments Develop and update individualized plans of care Monitor patient conditions and communicate changes to the appropriate care team Provide medication management, wound care, symptom management, and other skilled nursing services Educate patients and families regarding diagnoses, medications, treatments, and care plans Coordinate care with physicians, advanced practice providers, therapists, social workers, aides, and other members of the interdisciplinary team Complete accurate and timely clinical documentation Support patients and families through home health, hospice, and/or palliative care services Travel throughout an assigned Pennsylvania territory to complete scheduled patient visits Maintain compliance with applicable clinical, regulatory, and organizational standards What We're Looking For Active Registered Nurse (RN) license Home Health and/or Hospice nursing experience strongly preferred Experience providing care in patients' homes or other field-based environments Strong clinical assessment and care-coordination skills Ability to work independently while remaining connected to an interdisciplinary care team Strong communication, documentation, and time-management skills Comfortable managing a daily patient-visit schedule Reliable transportation and willingness to travel throughout an assigned service area Ability to provide compassionate, patient-centered care to individuals and families Ideal Background We especially want to hear from RNs with backgrounds in: Home Health | Hospice | Palliative Care | Visiting Nurse Services | Community Health | Case Management | Skilled Nursing |
