  • Registered Nurse to join our team at Lackawanna County Prison in Scranton. This role is ideal for an RN who is passionate about substance use recovery, continuity of care, and helping patients successfully transition back into the community. MAT Coordinator
  • Registered Nurse (RN) Part-Time | Weekend Overnight Shift Saturday & Sunday 11:00 PM
  • 7:00 AM 16 hours per week What You'll Do As the MAT Coordinator, you'll play a key role in supporting patients with substance use needs during incarceration and helping connect them to treatment resources after release.
Your responsibilities will include:
Coordinating daily patient care activities to ensure physician orders are completed and properly documented using POMR format Scheduling staff and patients to maintain effective patient care coverage Establishing communication with community programs to support continuity of care and streamline appointment scheduling Assisting with discharge planning and MAT intake referrals Collaborating with medical and behavioral health providers on patient treatment and release planning Supporting transitional care programs for chronically ill and high-needs patients Tracking and maintaining MAT program data, including: Individuals inducted onto MAT Individuals continued on MAT Release-day referrals for treatment Admissions into community-based programs MAT continuation rates after release What You Bring Current Registered Nurse (RN) license Current CPR certification Experience with electronic medical records (EMR) Strong communication, coordination, and documentation skills Passion for patient advocacy and recovery-focused care Why This Role Matters This position goes beyond traditional nursing care.
