What You'll Do:

Deliver skilled nursing care to patients in their homes throughout assigned Pittsburgh-area communities Develop and manage plans of care in collaboration with physicians and interdisciplinary partners Provide patient and family education that promotes independence and recovery Complete required documentation (OASIS, visit notes, orders) electronically using a provided laptop Contribute to high-quality, patient-centered outcomes while maintaining regulatory standards This Role Is a
