A Nurse Case Manager applies nursing expertise to coordinate a patient's medical care and provides advocacy to help them get the best and most affordable care. Develop care plans including discharge planning, coordinate delivery among providers, and teach patients and families how to follow the plan. They may also help patients navigate insurance and other healthcare bureaucracies and access related social services. May work with patients who have chronic illnesses or other complex medical needs. Works at a hospital or other healthcare facility; may follow patients throughout the span of their treatment.

Job Description

LTC Compliance Inc provides consultant pharmacist services to skilled nursing facilities. Our services include drug regimen reviews, nursing unit inspections, medpass audits, and education to long term care facilities. We are currently looking for an experienced nurse to work in conjunction with our pharmacists to perform audits of nursing units, at skilled nursing facilities including: 1. Monitoring for expired medication 2. Monitoring of medication storage 3. Compliance with state and federal rules and regulations 3. Medication room and cart audits 4. Observing nurses on medpass 5. Documentation of findings 6. Attending QAPI meetings Requires site visits to different nursing homes once a month. Candidate should be self motivated, reliable, able to meet deadlines, and must be an have excellent oral and written communication skills. Candidate must be detail oriented, and conscientious. Also must be willing and able to reach high cabinets and bend to low cart drawers. Experience in long term care facilities or pharmacy. Experience managing inventory a plus. Excellent customer service skills required.
Pay:
$42.00 - $50.00 per hour
