Description:
The Nurse Navigator serves as a vital link between our patients and clinical team, blending hands-on nursing care with advanced IV therapy administration and a genuine commitment to exceptional patient experience. You will coordinate continuity of care, perform clinical procedures, and help shape the future of our IV services program, all within a warm, collaborative practice that values your expertise and growth. What You'll Do Serve as a primary point of contact for patients, coordinating care, communicating procedural guidelines, and fostering strong, trusting relationships. Administer and monitor IV therapy services, including hydration, nutrient infusions, mistletoe, and ozone treatments, ensuring compliance with clinical guidelines. Schedule and oversee IV therapy appointments to ensure efficient, high-quality service delivery. Maintain accurate and timely documentation in patient records and electronic medical record systems. Identify and champion opportunities to continuously improve workflows and the patient experience. Uphold the highest standards of safety, confidentiality, and professionalism at all times.
Requirements:
Active RN license with appropriate Pennsylvania state licensure (required). Minimum of three years of clinical experience, including IV therapy administration. Exceptional patient care and interpersonal skills , you genuinely enjoy connecting with people. Strong organizational skills and meticulous attention to detail. Proficiency in clinical documentation and electronic medical record (EMR) systems. Collaborative, team-oriented mindset with a passion for continuous improvement. Background in functional medicine principles or willingness to learn Experience in customer service or patient relations (helpful).
