A Nurse Case Manager applies nursing expertise to coordinate a patient's medical care and provides advocacy to help them get the best and most affordable care. Develop care plans including discharge planning, coordinate delivery among providers, and teach patients and families how to follow the plan. They may also help patients navigate insurance and other healthcare bureaucracies and access related social services. May work with patients who have chronic illnesses or other complex medical needs. Works at a hospital or other healthcare facility; may follow patients throughout the span of their treatment.

Position Summary:
Case Manager (RN) coordinates a resident's skilled care according to physician orders and ensures communication of skilled care with the interdisciplinary team, resident and family. Position Responsibilities of a
Registered Nurse Case Manager:
    Pre Admission:
    Conducts on-site assessments, verifies payer source, completes pre admission screening form, pharmacy and med surge quotes
      Admission:
      Review and initiate orders, communicate with care community personnel, greet resident and family
      • Coordinate residents MDS's and Care Conferences
      • Participates in the QA process and "Nurse On-call" program
      • On-going communication regarding resident goals, progress and discharge planning
      • Assumes responsibility for maintaining his/her own professional competence
      • Oversees skilled residents care and works closely with the
      Office Manager Knowledge, Skills and Abilities:
      Graduation from an accredited school of Nursing. Current license as a Registered Nurse with the State Board of Nursing. CPR certification recommended. Benefits of for a
