$34.00/hr Milestone Centers, a subsidiary of Partners For Quality Inc., is a nonprofit, community-based human services agency supporting individuals with behavioral health challenges and intellectual & developmental disabilities in communities throughout Western Pennsylvania.Responsibilities:
Coordinate physical health related processes of the HCQU. Primary duties include, but are not limited to: Develop and implement training (face-to-face, on-line, webinar and home-study) which will increase competency and capacity of medical health care in the community serving developmentally disabled individuals. Assist with the review of health practices and policy development in areas defined by the Lead County and MOC. Assist in reviewing reports and investigations of deaths, injuries, accidents and hospitalizations for trends and outliers agency and county-wide as requested by county personnel. With state, regional, and other HCQU staff, analyze aggregate health patterns and trends. Maintain professional relationships with all Agency Providers, staff, Supports Coordinators and Case Managers, local health care providers, hospitals and hospice providers. Research, develop and present informational materials and products that will assist, educate, and support individuals with developmental disabilities as well as their caregivers/staff and community health professionals. Attend HCQU sponsored functions and activities and perform duties as requested by management team.CASH INCENTIVES
