Moorpark, CA Job Details Per diem $140 a day 15 hours ago Qualifications CPR Certification RN License BLS Certification Driver's License Full Job Description Access TLC is currently seeking a Registered Nurse (RN) to join our team in the CCT/CCA department. The Registered Nurse (RN) is responsible for assessing, and developing a person-centered care plan and evaluating each case and the services needed according to requirements of the California Community Transitions (CCT) program. This position involves a combination of in- person visits and remote work. The program is designed to assist Medi-Cal beneficiaries to remain in their community as an alternative to a skilled nursing facility and provides specified benefits to eligible seniors and persons with disabilities. The goal of the program is to facilitate a safe transition of Medi-Cal eligible persons from a nursing facility to a community home-like setting. This position is per diem and visit rate is $140 per visit depending on the type of visit. Mileage is also reimbursed. Access TLC is an accredited hospice and home health agency who provides unparalleled quality of service that allows patients to have a better quality of life. We have been serving Ventura, Santa Barbara, and Los Angeles counties for over 20 years.
Requirements include:
1. Registered nurse with current licensure to practice professional nursing in the State of California. 2. Have work experience that includes at least a minimum of 1000 hours experience in an acute care setting providing nursing care to patients with similar care needs and/or 2000 hours of experience in a home care setting providing nursing care to patients with similar care needs. 3. Demonstrates excellent observation, verbal and written communication skills. 4. Possess and maintains current CPR certification. 5. Valid California Driver's License with proof of a good Motor Vehicle Record. 6. Current proof of auto insurance with state minimum (or greater) coverage.
