Job Description

We are seeking a compassionate, dependable, and experienced Registered Nurse (RN) to provide nursing services and clinical oversight for individuals receiving services in HCS Host Home and Group Home settings. The Contract RN will conduct nursing assessments, monitor individuals' health needs, provide clinical guidance, and work closely with caregivers, direct-care staff, families, and the interdisciplinary team to ensure quality care. Responsibilities Conduct comprehensive nursing assessments for individuals receiving HCS services. Monitor individuals' health status and identify changes in condition. Assess medical and nursing needs and make appropriate recommendations. Develop, review, and update nursing care plans as needed. Provide nursing oversight and clinical support in Host Home and Group Home settings. Provide education and clinical guidance to caregivers and direct-care staff. Monitor medication administration and identify medication-related concerns. Communicate with physicians, pharmacies, families, guardians, caregivers, and other healthcare professionals. Follow up on illnesses, injuries, hospitalizations, and changes in medical condition. Complete accurate and timely nursing documentation and required assessments. Participate in interdisciplinary team meetings and assist with ISP development and implementation. Monitor health and safety needs of individuals. Ensure nursing services comply with applicable HCS regulations, policies, and standards of care. Travel to assigned Host Home and Group Home locations as needed. Qualifications Current, unrestricted Texas Registered Nurse (RN) license. Experience working with individuals with intellectual and developmental disabilities preferred. HCS, Host Home, Group Home, residential, home health, or community-based nursing experience preferred. Experience performing nursing assessments preferred. Strong clinical assessment, communication, documentation, and organizational skills. Ability to work independently. Reliable transportation. Current CPR/BLS certification preferred. Must meet applicable background check and employment requirements. Contract Position This is a Contract Registered Nurse position providing nursing assessments, clinical oversight, and healthcare support to individuals in HCS Host Home and Group Home settings.
Pay:
