Home Health Care RN Case Manager

  • Part-time Job Overview Cornerstone Home Health, Inc.
has a PRN position for an RN. Home Health experience preferred. Applicants must have proven experience with Skilled Nursing visits, all OASIS to include RECERT visits, ROC visits, TRANSFERS, DC visits, and SOC's. Knowledge of wound care, IV therapy, and medication management a plus. Applicants should be able to schedule and manage time to complete assignments with required time frames. Salary is per visit, plus trip fees. Rate ranges from $45.00 to $125.00 per visit-experience determinate. Applicants need to have availability as early as 8:00 am and as late as 6pm and must be available for attendance at Case Conference. Responsibilities Conduct thorough patient assessments, including vital signs, medical history, and current health status, to develop individualized care plans. Manage post-acute, outpatient, and home-based patient care by coordinating services and monitoring progress. Collaborate with physicians, therapists, and other healthcare providers to ensure continuity of care and effective discharge planning. Perform diagnostic evaluations and oversee case management activities to optimize treatment outcomes. Educate patients and families on disease management, medication adherence, and health maintenance strategies. Maintain accurate medical records in compliance with HIPAA regulations and facility policies. Utilize clinical judgment to identify urgent needs or complications, escalating cases as necessary for prompt intervention. Requirements Valid RN license with current registration in Oklahoma. Critical care experience including ICU, emergency room, or hospital medicine settings. Knowledge of infectious disease care, geriatrics, behavioral health, and post-acute care environments. Strong understanding of medical terminology, physiology, and diagnostic evaluation techniques. Proven ability in patient assessment, discharge planning, and utilization management. Excellent communication skills for effective patient service and interdisciplinary collaboration. This role is ideal for motivated nurses eager to leverage their hospital experience.
