Manage complex wound care procedures such as suturing, ostomy care, and dressing changes while maintaining sterile environments. Perform specialized treatments including botulinum toxin injections for applicable conditions and assist with procedures like catheterization and tube feeding. Utilize electronic health record (EHR) systems such as Epic or Cerner to document patient interactions accurately and efficiently. Collaborate with interdisciplinary teams to develop discharge planning strategies, coordinate long-term care, and facilitate case management for patients with chronic conditions or developmental disabilities. Provide education on disease management, medication administration, nutrition, pain management, and health coaching tailored to individual needs across geriatrics, pediatrics, behavioral health, and hospice care. Skills Proven experience working with diverse patient populations in settings such as home care, nursing homes, assisted living facilities, or hospitals. Strong knowledge of medical terminology, anatomy, physiology, and diagnostic evaluation methods including laboratory experience and medical imaging interpretation. Proficiency in EMR/EHR systems like Athenahealth or eClinicalWorks; familiarity with ICD coding (ICD-9/ICD-10) is preferred. Critical care experience including ICU or trauma center background; experience with ventilator management and acute pain management is highly valued. Skilled in performing invasive procedures such as venipuncture, phlebotomy, IV insertion, specimen collection/processing, and aseptic techniques. Ability to triage effectively in urgent situations; experience working within trauma medicine or emergency medicine environments is advantageous. Excellent communication skills for patient education, family interactions, documentation accuracy in medical records while maintaining HIPAA compliance.
