A Registered Nurse provides medical care and treatment, educates patients about their conditions, and provides emotional support to patients and families. Works in hospitals, schools, clinics or community health centers. Works under the supervision of a physician or other specialist, and serves as the primary point of care for patients in a hospital setting. May specialize in emergency room work, or treatment for a particular condition, or specialize in working with infants, the elderly or other patient groups. May work as an advanced practice specialists and prescribe medications in addition to offering specialty care.
Position Summary We are seeking a compassionate, autonomous, and detail-oriented Licensed Vocational Nurse (LVN) to join our home health clinical team on a per diem basis. In this role, you will provide high-quality skilled nursing care to patients in the comfort of their homes under the supervision of a Registered Nurse (RN). Key Responsibilities Direct Patient Care:
Perform skilled nursing visits in patient homes according to the established Plan of Care (POC).
Treatments & Procedures:
Administer medications, perform wound care, dressing changes, catheter care, routine lab draws, and vital signs monitoring.
Patient Education:
Educate patients and family caregivers on disease management, medication adherence, post-surgical care, and safety precautions.
Timely Documentation:
Complete and submit accurate clinical visit notes via Electronic Health Record (EHR) within 24 hours of service delivery.
Care Coordination:
Report changes in patient condition promptly to the Case Manager (RN), attending physician, and interdisciplinary team.
Compliance:
Adhere to all local, state, and federal regulations, as well as agency policies and HIPAA standards.
Active, unencumbered Licensed Vocational Nurse (LVN) license in the state of practice. Current BLS/CPR Certification from the American Heart Association.
Experience:
Minimum 1 year of clinical experience as an LVN (Home Health, Hospice, or Post-Acute care experience strongly preferred).
Key Competencies:
Proficient in wound care, medication administration, and basic assessment skills. Familiarity with Electronic Medical Record (EMR) systems (e.g., WellSky/Kinnser, Homecare Homebase, or similar).
Transportation & Logistics:
Valid driver's license, reliable transportation, and active auto insurance. Ability to work independently and manage daily travel schedules effectively.
Compensation & Benefits Competitive Pay:
$40.00 - $55.00 per hour / per visit equivalent.
