Job Summary Mass General Brigham Population Health Services Organization (PHSO) is seeking an experienced Advanced Practice Provider (APP) to join an innovative, multidisciplinary team focused on improving outcomes for Medicare beneficiaries undergoing major surgical procedures. This role supports the Centers for Medicare & Medicaid Services (CMS) Transforming Episode Accountability Model (TEAM), an episode-based care model designed to enhance quality, coordination, and patient experience while reducing unnecessary utilization and readmissions. The APP will play a critical role in guiding patients through the full surgical journey, from pre-operative assessment and optimization through recovery and post-discharge follow-up. Working collaboratively with physicians, care coordinators, therapists, and other clinical partners, the APP will provide both virtual and in-person care, identify and mitigate clinical risks, address quality care gaps, and support safe, successful transitions across care settings. This is a senior-level APP opportunity that combines direct patient care with clinical leadership responsibilities. The APP will primarily support geriatric patients undergoing orthopedic, cardiac and major bowel surgery, providing comprehensive pre-operative optimization, perioperative support, and post-discharge follow-up throughout the surgical episode.   • and high-risk patients, the APP will serve as a clinical resource for the broader TEAM program, helping to refine care pathways, support program development, mentor colleagues, and advance evidence-based practices that improve patient outcomes, care coordination, and continuity across the Mass General Brigham system.
We are seeking a full-time, 40-hour Nurse Practitioner or Physician Assistant to join our team supporting patients through the perioperative care continuum. The APP will be required to conduct seven patient visits per day through a combination of in-person and virtual visits. The anticipated weekly schedule is as follows:
    Monday:
    In-person patient visits
      Tuesday:
      Alternating remote administrative day and onsite team meeting at Assembly Row in Somerville
        Wednesday:
        Virtual patient visits
          Thursday:
          In-person patient visits
            Friday:
            In-person patient visits To support patient access and minimize travel, in-person clinical sessions will be based on the selected candidate's geographic location within the Mass General Brigham footprint.

            The NP is responsible for assessing and managing various patient populations, including those with medical and/or surgical conditions, as well as providing diagnostic and therapeutic interventions, developing appropriate plans of care, and conducting ongoing evaluations.             • Performs complete histories and physical examinations.
            • Orders, interprets, and evaluates appropriate laboratory and diagnostic tests.
            • Develops appropriate plans of care and follow-up based on the outcomes of diagnostic, laboratory, and physical examination findings.
            • Orders medications and writes prescriptions according to organizational and regulatory policies and procedures.
            • Consistently provides high-quality and timely documentation, including admission and progress notes, procedure notes, operative notes, and discharge summaries.
