Job Overview Join our dynamic healthcare team as a PRN Registered Nurse (RN) or Licensed Vocational Nurse (LVN), where your expertise will make a meaningful difference in patient care across diverse settings. Responsibilities Perform comprehensive patient assessments, including vital signs, physical examinations, and physiological evaluations to inform care plans. Administer medications and injections accurately, including IV infusions and sterile procedures, following all safety protocols. Document all patient interactions meticulously within electronic health record (EHR) systems such as Kinnser, ensuring compliance with HIPAA regulations. Provide specialized care for individuals with complex conditions such as Alzheimer's disease, dementia, or those requiring memory or behavioral health management. Assist with diagnostic evaluations including specimen collection, processing, and interpretation of results using medical imaging and laboratory techniques. Support discharge planning and case management efforts to ensure seamless transitions across care settings. Engage in patient education and health coaching to promote wellness and adherence to treatment plans. Qualifications Valid RN or LVN license in good standing; current CPR/BLS certification required. Proven experience working in Home Health or Hospice. Familiarity with EMR/EHR systems including Kinnser. Demonstrated knowledge of medical terminology, anatomy, physiology, and pharmacology relevant to acute and chronic patient care. Experience with specialized nursing procedures such as IV insertion, catheterization, suturing, tube feeding, and sterile techniques. Ability to perform physical assessments and diagnostic evaluations accurately; experience with ventilator management and airway support is advantageous. Strong critical thinking skills with the ability to triage effectively in urgent situations; familiarity with infectious disease control and infection prevention protocols. Compassionate approach to working with diverse populations including pediatrics, geriatrics, individuals with disabilities or developmental disorders such as autism or Alzheimer's disease. Excellent communication skills for clear documentation and collaboration within multidisciplinary teams; commitment to maintaining HIPAA compliance at all times.
