This is a field-based role with virtual team collaboration. Travelers will complete approximately 2 days of onsite orientation in Oak Brook, IL with Advocate@Home before transitioning to a fully remote field-based schedule within the assigned territory.

Patient Population Adult and geriatric general Med/Surg patients Current team census: approximately 200 patients Expected productivity: 5-6 patient visits per day 1:1 patient care Required Qualifications Minimum 2 years of Home Health PT experience Experience with adult/geriatric patients Active Illinois PT License BLS Certification (AHA required) Master's Degree in Physical Therapy Valid Driver's License Active auto insurance and reliable transportation Recent Home Health experience strongly preferred First-time travelers considered with strong experience Required Skills Home care/Home Health experience Oasis documentation experience

experience Ability to assist with ADLs and adaptive equipment Strong interdisciplinary collaboration skills with RN, OT, Speech, and other care team members
