POSITION DESCRIPTION
Under the general direction and supervision of an RN2 (community) or RN Supervisor 1, independently provides direct nursing care to individuals in accordance with Article 139 of the Education Law; supervises nursing care activities; and provides clinical supervision and training to Licensed Practical Nurses, and direct care staff.PRIMARY DUTIES AND RESPONSIBILITIES
- Conduct nursing assessments; and obtain, record, and update individuals' health information and vital signs, using standard instruments and procedures.
- Administer medications and nursing related treatments to individuals as indicated.
- Prepare, implement, and modify plans of nursing service; prepares individuals for and/or assists with examinations and treatments; and administers medications and nursing procedures to individuals, as needed.
- Observe, monitor, and record individuals' symptoms, conditions, and care, including diet and physical activity; and identifies and reports individuals' responses to medication or changes in health status.
- Make clinical rounds to assess individuals' nursing care needs and observe and participate in the provision of nursing services. Provides health teaching, counseling, and education to individuals, families, and other groups; and develops health improvement programs.
- Implement infection control protocols and procedures and provide related consultation and instruction to staff.
- Maintain and update charts, records, and other documentation, consistent with agency, State, and federal requirements; and may collect data and perform related activities to support medical research.
- Monitor inventories and ensure that standards for medications, solutions, supplies, and equipment are maintained to meet nursing care needs.
- Act as liaison to outside agencies and providers to ensure continuity of care and the necessary follow-up for individuals, advocates for individuals' needs.
- As needed, act as a triage nurse; accepting calls from the six-county catchment area and providing nursing guidance and training to direct care staff/LPNs.
- Assist with electronic medication administration record (e_MAR) / EHR training, support, and compliance.
- Perform other duties as assigned; details to be discussed during interview.

Additional CommentsThis title is part of the New York Hiring for Emergency Limited Placement Statewide Program (NY HELPS). HELPS Program titles may be filled via a non-competitive appointment. This means that you do not need to take an exam to qualify, but you do need to meet the minimum qualifications of the title. At a future date (within one year of permanent appointment), employees hired underNY HELPS
are expected to have their permanent non-competitive employment status converted to permanent competitive status. You will not have to take an exam to gain permanent competitive status.- Please note that starting salary for this title reflects the increased hiring salary and geographic pay differential. Posted Hiring Salary includes the sum of the statutory hiring rate for PS&T Grade 18 ($66,951), Increased Hiring Salary ($9,196), and Geographic Pay Differential ($20,000). Geographic Pay Differential is added to the job rate ($85,138).
- If you are currently a New York State employee serving in a permanent competitive qualifying title and eligible for a transfer via Section 70.1 of the Civil Service Law, you may be appointed to this title as a permanent competitive employee.
- The assignment of PS&T employees will be made in accordance with any agreed upon Memorandum of Understanding between PEF and the local DDSO regarding the posting of job vacancies.
