Job Description

Compensation:
$80,000 - $90,000 annual salary Annual bonus

Key Responsibilities:
Complete an initial assessment of the patient to determine home care needs through phone calls, video calls, and home visits Develop a plan of care that establishes goals and actions based on initial assessment Educate the attending CNA on the plan of care and techniques to meet the patient's needs Conduct periodic supervisory visits to observe client health and record patient's reaction to treatment, patient's progress, and any changes in patient's condition
Qualifications:
At least 3 years of RN experience - hands on clinical experience Current Colorado RN license

Colorado Family Caregivers is a family-owned company that helps families of children with intellectual and developmental disabilities provide care to their loved ones in the client's own home. Our Home Health program employs parent-CNAs to provide these services for their family members. Our RNs help the parent-CNAs understand the plan of care and periodically check-in to ensure the client's needs are being taken care of by the parent-CNA.
