We are seeking a dedicated Registered Nurse (RN) to join our team and provide quality hospice services to patients in their homes throughout Cook, Kane, DuPage, Lake, and Will Counties. Responsibilities Conduct admissions and initial assessments for new hospice patients Provide PRN visits based on clinical needs and patient requests Respond to death calls with professionalism and compassion Perform routine and cover visits to support the interdisciplinary team Document visits and assessments accurately using EMR Communicate with patients, families, and care teams effectively and empathetically Qualifications Current Registered Nurse (RN) license in Illinois (required) Hospice or home health experience preferred Excellent clinical judgment and documentation skills Must have reliable transportation , valid driver's license, and auto insurance Ability to work independently and manage time efficiently Comfortable with travel across multiple counties Benefits Competitive salary based on experience Paid time off and holidays Supportive team environment Apply today to join a caring, professional team committed to making a difference in the lives of patients and families.
